CSSAF Purchase Request Form

Before purchasing any item for your CSSAF project, you are required to complete the CSSAF Purchase Request Form. After the request is reviewed, you will be notified via email to alert you that your request to purchase an item is approved, denied, or pending because of a need for more information. You must fill out a form for each item you wish to purchase.

CSSAF Project Name

Your Full Name (first, middle initial, last)

Your Net ID

Item you are requesting to purchase for your CSSAF project

Detailed description of the item *When applicable specify brand, model number, dimensions, quantity, price per unit, etc.

Specify preferred vendor *Include link to product on vendor page if possible.

Date item is needed (month/day/year)

What purchasing method do you plan to utilize?

Method A: Purchase with personal funds and submit reimbursement documentation
Method B: Work with faculty or staff sponsor to purchase using University Pcard
