Thank you for your interest in submitting an activity for the IPE Passport Curriculum facilitated by the Center for Interprofessional Practice, Education, and Research (CIPER). The application process has 2 steps, the written application (5 sections) and then a short meeting with CIPER's Curriculum Committee. For more detailed information, please see the Policy and Procedure document titled, IPE Passport Activity Approval Process, found on the CIPER website. 

CED 400 course Prerequisite
Please be aware of the following prerequisite: Completion of the course CED 400: Introduction to Collaborative Care. We require at least one faculty facilitator on your submission team to have completed the CED 400 course prior to implementing your activity. We encourage you to complete CED 400 course by contacting IPEPassport@creighton.edu Your course completion will assist you in developing your interprofessional learning objectives and submitting this application effectively. 

Application Directions 
Please ensure you answer all questions thoroughly. The are 5 sections to this application. You can go back into a survey response and edit it after it’s been submitted. Please email IPEPassport@creighton.edu for a retake survey link. This is useful if you want to change your answer to a question, or if your application was incomplete/closed because of inactivity, but you would still like to finish the application.
 
Post Completion
CIPER's Curriculum Committee will review your submission and provide a response according to the IPE Passport submission timeline.
